Mine Safety Training and Certification Testing Frequently Asked Questions

To schedule safety training, complete and submit the Request to Schedule Training form. For more information, contact the Mine Safety Training Center in Cadiz, OH at (740) 942-9150.

To be placed on a roster for surface or underground certification examinations, complete and mail a notarized application to the Ohio Mine Safety Training Center in Cadiz, OH marking the envelope 'Attention: Certifications'. Individuals then will be notified, in advance, of the time of the examination.
